Once you have completed the form you may return it to our office. You may drop it off at our office, mail it, or fax.

Fees and Payment
It is our goal to keep the services we provide as affordable as possible for everyone. We ask that you help by paying for your care within 30 days from the date of service or insurance payment. Our office staff is available Monday-Friday 8:00-5:00 to answer any question or to help with financial arrangements.
Insurance
Please bring your card with you to every visit. Health Insurance and other benefits such as Medicare, Medicaid, Health Savings Accounts (HSA's), etc. may cover most of the cost of your treatment but most require that you share financial responsibility. You may be required to pay your co-payment, deductible, or coinsurance. Our office requires that you pay your co-payment ath the time of service.

Multiplan First Health Personal Care
If you do not have insurance you might want to check into the Illinois Breast and Cervical Cancer Progam. They offer free Breast and Cervical screenings to uninsured women. www.cancerscreening.illinois.gov
